Making an Appointment:

Our Appointment specialist will facilitate your visit from start to finish allowing your health to be first priority.

Our intake staff will collect all pertinent information for your visit with us. Provide your insurance information by phone, and a staff member will obtain the necessary verifications before your appointment.

It is important we receive your medical records, labs, pathologies, operative reports, or radiology images prior to your first appointment. We will work diligently on acquiring this information before your visit.

Designed to make your experience with Hematology Oncology Associates as convenient and pleasant as possible, an appointment specialist will contact you after your records have been reviewed by our doctors to schedule your appointment.

Initial Visit
Please arrive at our office at your scheduled appointment time. It is not necessary to arrive early to complete paperwork. Please remember to bring your insurance information/cards with you including a referral, if required, and a Driver’s License or other form of identification.

Bring a list of all medications that you are currently taking (including over-the -counter remedies, vitamins and herbal supplements.)

Write down any questions or concerns that you may have to discuss with your physician. If you wish, bring someone with you to help listen to your physician’s comments and responses to your questions.

Each time you come to Hematology Oncology Associates, you will sign-in by filling in your name, time of appointment and Doctor you are seeing on the sign-in sheet in the waiting room. Shortly, you will be called back by a Medical Assistant or R.N. to have your vitals taken and labs drawn. In the meantime, take a seat and relax. We strive to keep waiting times to a minimum, but please realize that delays can occur due to emergencies and unforeseen patient needs.

If your insurance, address or other demographic information has changed since your last appointment, please notify us at the time of check-in.

Your next appointment and any outside tests that are needed will be scheduled before you leave our office.
